Cigarette License Applications

The Wisconsin DOR has recently updated their Cigarette License Applications.

All applicants must complete the CTV-100 as well as the CTV-101.

Corporations & LLC must complete both the CTV-100 & CTV-101 as well as the CTV-102.

If you have any questions regarding the applications themselves, you must contact the DOR.
